A vaccination certificate is generated in VAMS (Vaccine Administration Management System) when a healthcare professional records a COVID-19 vaccine dose for a recipient in VAMS.

VAMS will update vaccination certificates automatically if healthcare professionals make any necessary changes to the recipient’s vaccination records in VAMS.

Details Mentioned in Vaccination Certificate

  • The vaccination certificate will have details about when the COVID-19 vaccine was given, which company made the vaccine, the batch number, and the name of the clinic where the vaccine was given.
  • The vaccination certificate only shows the COVID-19 vaccine dose(s) recorded in VAMS.

How Can We Use Vaccination Certificate?

  • Once the recipient has received all the necessary doses of the COVID-19 vaccine, they will be given a certificate. This certificate will serve as their official COVID-19 vaccination record, which will be documented in VAMS.
  • Recipients can easily access their vaccination certificate at any time by using the Recipient Portal.

How to Download Your Vaccine Certificate?

  • Open the Recipient Portal to access your account. To get started, go to your jurisdiction’s VAMS landing page. If you already have a registered VAMS Recipient Account, click on “VAMS Recipient Login”. If you don’t have a registered account, click on “Proceed as Guest”.
    • If you chose “VAMS Recipient Login,” please provide the email and password that are linked to your VAMS Recipient Account.
    • If you chose to proceed as a guest, click on “Manage Appointments”.Please provide all the necessary information to identify your Recipient Profile. You will receive a Confirmation Code through your preferred method of contact.Please enter the Confirmation Code into the empty field in the Verification Code pop-up window. Click the “Verify” button.
  • To access the Recipient Portal, click on the My Appointments tab located at the top of the screen.
  • To access your vaccination certificate, simply click on the blue text that is hyperlinked. It will say “View Your Vaccination Certificate.”
  • You will see a pop-up window showing your Certificate of COVID-19 Vaccination.

How to Generate Vaccination Certificate QR Code?

  • To generate a QR code, select “Generate QR Code” from the pop-up window that displays your Certificate of COVID-19 Vaccination.
  • VAMS will open a new window for you. To set up a PIN for accessing your vaccination certificate, please follow the instructions in this window.
    • The PIN needs to be 6–8 alphanumeric characters.
  • Click Save.
  • VAMS will bring you back to your Recipient Portal.
  • To access your vaccination certificate, simply click on the blue text that says “View Your Vaccination Certificate”. The QR code will appear in a separate window along with your COVID-19 Vaccination Certificate.
    • To print the QR code, click on the Print option located below the QR code.
    • To change the PIN for accessing your vaccination certificate, choose “Change QR Code PIN”. Simply follow the same steps you used to initially set up your QR Code PIN.
  • Your QR code is ready to be scanned!
    • When scanned by an external user, the QR code will generate a link to an external site. The page will prompt you to enter your QR Code PIN. Enter your PIN and click Continue. Once you enter your PIN, vaccination details will display.

About Vaccination Certificates

  • Please be aware that if you haven’t scheduled and received your vaccinations through VAMS, the system cannot currently give you a vaccination certification or vaccine verification QR code. If you have scheduled and received your vaccinations through VAMS and are having difficulty getting your vaccine certificate, please contact the POC (Point of Contact) for your jurisdiction.
